 New Patient Instructions
 
 
To expedite the registration process, arrive at least 15 minutes prior to your first appointment.
 
Bring  your insurance card and a picture ID. 
 
Please bring your current medications, supplements, and vitamins list.
 
If you arrive late for your appointment you may be asked to reschedule.
 
  • If you are being treated for hypertension or diabetes, please bring a log of your blood pressure or blood sugar readings.
 
  • Get blood work drawn one week prior to your appointment date, so test results will be ready for us to discuss during your appointment.

  • Fasting blood work is strongly advised for getting a better picture of your sugar,cholesterol and thyroid levels.  In other words, you should not have eaten foods, drinks etc at least during the 10 hours prior to blood drawing.
 
  • For vaccination updates, call ahead to make sure that we have the vaccine in stock.  Bring your vaccination record if you have not already done so.
 
 
For Every Office Visit
 
  • Always bring  your insurance card for our receptionist to confirm the accuracy of your current insurance carrier.
 
  • Advise us of any changes in your address, telephone numbers, home and workplace information.
 
  • Bring a list of all medications, supplements and vitamins that you are currently taking.
 
  • Review ahead of time the medications that you need to refill.
 
  • Advise us of any changes in your medical condition since your last appointment with us.
 
 
No Show Policy
 
  • If  you are unable to come for your appt, please call 24 hours in advance to reschedule or cancel your appointment. There is a $25.00 no show fee.
 
  • If you have a no show fee, you are expected to pay this fee before your next appointment.
