 Payment and Insurance Policies - We expect payments of all fees: co payments and deductables at the time of your visit.
- We accept Cash, Visa, MasterCard, American Express, and Discover.
- If you do not have your insurance information with you at the time of your visit, you will be expected to pay in full for services rendered on that day.
- If this policy presents a hardship to you, please discuss your situation with our office prior to your appointment by calling 225-757-6031
- It is your responsibility to notify us of any changes made to your Insurance Plan.
No Show Policy - Please call ahead at least 24 hours to reschedule or cancel your appt if your are unable to come for the scheduled visit to avoid the no show fee $25.00.
- If you have a no show fee, yor are expected to pay this fee before your next visit with the doctor.
Patient Referral Policy - In some cases you may need a referral to a specialist for management of your Medical condition.
- If you are covered by an insurance plan that requires a referral, it usually takes about 3-4 days to get the necessary authorization from your insurance company.
- Once it is obtained, our staff may notify you to make an appointment at your convenience, date and time.
Medication Requests - Most medications are given for several months, but some medications require closer follow up.
- Blood tests will need to be done to adjust the dosages and to prevent negative side effects of some medications.
- Once you start your last refill you need to contact our office for an appointment.
- Please remember to allow 24-48 hours for the processing of all medication refill requests.
- Please have your pharmacy contact us for refills. If you need to be seen in order to get any refills you will be notified.
- As the standard of care, we cannot call in antibiotics, narcotics, or tranquilizers without seeing you in the clinic. We do not prescribe medications for the conditions that you have not been treated in this office.
Emergencies and After Hours - During the week days (Monday- Thursday) Dr. Thomas is on call 24 hours a day for urgent medical problems.
- During the Weekend, you will be given the telephone number of a Doctor on call with instructions on contacting him.
- If you have a life threating emergency, dial 911 immediately for medical assistance or go to the nearest emergency room.
- The on call doctor will not address your chronic medical problems or your prescription refill requests.
